Fresenius Medical Care Health Plan and ESRD
Many Medicare Advantage Plans, Medigap Supplemental plans and private insurance plans will not offer coverage to people who have End Stage Renal Disease (ESRD). ESRD is a condition that normally follows chronic kidney disease in which the kidneys fail to rid the body of wastes. Often dialysis and/or a transplant are needed to treat it.
Over 50,000 people are diagnosed with end-stage renal disease (ESRD). Fresenius Health Partners is open exclusively to these patients. Fresenius offers a Medicare Advantage Health Plan specifically created to improve the coordination of all aspects of care for a dialysis patient.
It is important to note that Fresenius Health Partners is a subsidiary of Fresenius Medical Care, a large renal provider with access to programs like vascular access surveillance, nutrition programs, pharmacy services, and renal-specific laboratory services.
Plan Benefits
Fresenius will be offering a PPO/SNP Medicare Advantage Plan, meaning it is a Special Needs Plan via a Preferred Provider Organization. Some of the coordinated care benefits that will be offered under this policy include:
- KidneyTel® Program with a nurse that calls regularly to offer support, education and advice to policyholders.
- Nutritional support with recipes that are 'kidney-friendly.'
- Educational materials that are mailed to the patient and reviewed by the nurse;
- Diabetic support
Plan Details
Prescription Drug Benefit information, description of conditions and limitations, premiums and cost sharing information has not yet been approved by CMS, the Centers for Medicare and Medicaid Services for the upcoming year.
