Bravo Health

Bravo Health, formerly Elder Health, provides Medicare Advantage plans in Delaware, Maryland, New Jersey, Pennsylvania, Texas, and Washington, D. C. as well as Part D Prescription Drug Plans* in 43 states. In business for nearly 15 years, Bravo provides benefits for over 350,000 members.

Based in Maryland, Bravo has over 30,000 contracted providers. Their business seems to be more focused on Medicare Advantage plans than other insurers in this industry.

Plan Benefits

Bravo Health offers several plan options:

  • Medicare Advantage plans with HMO service;
  • Special Needs Plans (SNP) for those receiving institutional care, those with chronic health conditions, or those with dual eligibility, meaning they are eligible for both Medicare and Medicaid;
  • Point-of-Service (POS) plans which provide limited out-of-network features; and
  • Preferred Provider Organization (PPO) plans with out-of-network features.

The details and costs of Bravo's most popular plans are listed below.

Bravo Classic HMO

Bravo Classic is a Health Maintenance Organization (HMO) plan that covers both Medicare Part B prescription drugs and Medicare Part D prescription drugs.

Benefits and costs include:

  • A $6,700 out-of-pocket limit, which includes only Medicare-covered services.
  • You must go to network doctors, specialists, and hospitals.
  • No referral required for network doctors, specialists, and hospitals.
  • Inpatient hospital stays: Plan covers 90 days each benefit period. Days 1 – 7: $250 co-pay per day; Days 8 – 90: $0 co-pay per day (less expensive than traditional Medicare copayments, which are currently Days 1 - 60: $1100 deductible Days 61 - 90: $275 per day)
  • Skilled nursing facility: 100 days per benefit period:Days 1 – 10: $0 co-pay per day; Days 11 – 20: $50 co-pay per day; Days 21 – 100: $110 co-pay per day, no prior hospital stay required. These benefits offer a less costly and restrictive alternative to traditional Medicare, in which the 2010 the amounts for each benefit period after at least a 3-day covered hospital stay were Days 1 - 20: $0 per day Days 21 - 100: $137.50 per day. These amounts may change for 2011.
  • No additional premium for this plan, just the cost of traditional Medicare's monthly premium.

Bravo Premier Plus

Bravo Premier Plus is a Medicare Advantage Health Maintenance Organization (HMO), with a point-of-service option (POS).

This plan:

  • Allows you to see doctors outside of Bravo Health's network, although additional costs may apply;
  • Covers both Medicare Part B prescription drugs and Medicare Part D prescription drugs.
  • In and Out-of-Network services have a $6,700 out-of-pocket limit.
  • No referrals required for network doctors, specialists, and hospitals.
  • Inpatient hospital stays:90 days covered each benefit period: Days 1 – 7: $200 co-pay per day; Days 8 – 90: $0 co-pay per day (less expensive than traditional Medicare copayments, which are currently Days 1 - 60: $1100 deductible; Days 61 - 90: $275 per day)
  • Skilled Nursing Facilities:100 days per benefit period:Days 1 – 10: $0 co-pay per day; Days 11 – 20: $50 co-pay per day; Days 21 – 100: $110 co-pay per day. Bravo Freedom

Bravo Freedom is a Preferred Provider Organization, or PPO plan, that gives you access to an extensive group of medical providers and allows you to see doctors both in and out of our network. Part D Prescription Drug coverage is also covered in this plan.

  • In and Out-of-Network services have a $6,700 out-of-pocket limit.
  • No referrals required for network doctors, specialists, and hospitals.
  • Inpatient hospital stays:90 days for each benefit period Days 1 - 7: $250 co-pay per day; Days 8 - 90: $0 co-pay per day;
  • Skilled Nursing Facilities:100 days per benefit period. No prior hospital stay is required. Days 1 - 10: $0 co-pay per day; Days 11 - 20: $50 co-pay per day; Days 21 - 100: $110 co-pay per day. These benefits offer a less costly and restrictive alternative to traditional Medicare, as in 2010 the amounts for each benefit period after at least a 3-day covered hospital stay were Days 1 - 20: $0 per day Days 21 - 100: $137.50 per day. These amounts will change for 2011.
  • Out-of-Network facilities incur 20% of the cost for each SNF stay. $69 monthly plan premium in addition to your monthly Medicare Part B premium.
